Publication Permission for OPAL 2009

This document grants permission for paper publication in OPAL 2009.

I am (We are) the author(s) of the following paper, which has been
accepted for publication in the Proceedings of OPAL 2009, 1st International
Workshop on Dynamic Modeling of the Oral, Pharyngeal and Laryngeal Complex
for Biomedical Applications (OPAL-09):

By attaching our names to and sending this document, we give permission
to The University of British Columbia, OPAL 2009, and the organizers of
OPAL 2009 to publish our paper as part of the OPAL 2009 Proceedings,
which will be made widely available in print and electronic form. In
addition, we grant permission to The University of British Columbia 
and to the OPAL organization to post our paper with no time limit on a 
server where anyone can freely download a personal electronic copy of 
our paper. We understand that we will not receive any payment in return 
for granting these publication and distribution rights.

In addition, we have read and agree to the Copyright Notice on the first 
page of our paper, which says:

    Permission to make digital or hard copies of all or 
    part of this work for personal or classroom use is 
    granted without fee provided that copies are not 
    made or distributed for profit or commercial 
    advantage and that copies bear this notice and the
